Qualifications of a Medical Review Officer
According to the HHS Mandatory Guidelines for Federal Workplace Drug Testing Programs, an MRO must:
– Be a licensed physician holding either a Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) degree.
– Have extensive knowledge regarding the pharmacology and toxicology of illicit drugs.
– Be trained in the collection procedures for Federal agency specimens, interpretation of test results, chain of custody, and recordkeeping requirements.
– Pass an examination administered by an HHS-approved organization.
Ensure that specimens reported as negative or as negative and dilute are properly reviewed (i.e., at least 5% personally and the remainder by staff under the MRO’s direct, personal supervision) and reported to the appropriate regulatory Federal DOT administration, or US Coast Guard.

Make a Determination
- Ensure the review of the chain of custody form on all specimen collections for the purposes of determining whether there is a problem that may cause a test to be cancelled. As an MRO we are not required to review laboratory internal chain of custody documentation.
Provide feedback to employers, collection sites and laboratories regarding performance issues where necessary; and report to and consulting with the relevant DOT agency when you wish DOT assistance in resolving any program issue. Employers and/or service agents are prohibited from limiting or attempting to limit the MRO's access to DOT for this purpose as well as from retaliating in any way against an MRO for discussing drug testing issues with DOT.

Non-Negatives Outcomes (Prescription)
- MROs may inform the individual responsible for certifying that the donor is qualified to perform that job that the donor is taking a medication that is restricted for an individual in that occupation or that the medication may affect the individual’s ability to perform a safety sensitive occupation.
If side effects from a prescribed medication could affect the safety of a donor's work, the Medical Review Officer (MRO) must determine the appropriate course of action regarding this information. While the Mandatory Guidelines specify that an MRO should confirm a drug test as negative if the donor has a valid prescription, it is advisable for the MRO to communicate with the prescribing physician. This dialogue can help assess how the medication might influence the safety of the donor's work duties.

Donor Rights to Information
- Upon a written request facilitated by the Medical Review Officer (MRO) and the Federal agency an employee undergoing a drug test is entitled to access all records pertaining to their test including documentation related to the test results any certification processes reviews or revocations.
Additionally, either the donor or the Federal agency may occasionally ask the laboratory for an inclusive set of analytical data, chain of custody records, and other administrative documents linked to the testing of a specific specimen. This compilation of documents is sometimes referred to as a "documentation package," "data package," or "litigation package," and all requests for such information must be channeled through the MRO to the testing facility.

Positives & Possible Designations
- Invalid Adulterated Dilute Substituted
A positive will be rendered with the specimen’s immunoassay result was at or above the initial test cutoff for the drug class and the specimen’s confirmatory drug test result (i.e., on a separate [fractional part of a specimen used for testing, representing the whole specimen]) was at or above the confirmatory test cutoff for the specific drug/drug metabolite.
